BACKGROUND: Since 1952, Beacon Group has been creating opportunities for people with disabilities across Southern Arizona, Phoenix and beyond. As a registered 501(c)(3) non-profit, Beacon serves over 2000 people with disabilities each year. Beacon provides job training and placement, supported and center-based employment, and day treatment programs to assist the more than two-thirds of working-age people with disabilities who do not have a job. POSITION INTRODUCTION: The primary objective of Intake Specialists at Beacon Group is to ensure all referral packets are received completed with accurate and timely documentation and filed into the Electronic Health Record System.

RESPONSIBILITIES:
- Responsible for ensuring that all referrals from Vocational Rehabilitation and Behavioral Health funding sources are processed within the timeframes as determined by the supervisor.
- Follow-up with referral agencies to ensure complete packets.
- Scheduling intakes and tours with referral partners and referred clients on behalf of Beacon Employment Specialists, Vocational
- Instructors, Peer Support Specialists, and others.
- Complete and renew all trainings and certifications including, but not limited to, Workforce Development Core Competency
- Training, Relias Trainings, and Rehabilitation New Service Orientation.
- Collaboratively work with clinical mental health teams as needed.
- Follow all referral partner and Beacon Group rules and regulations regarding service provision.
CUSTOMER SERVICE:
- Provide program information when inquiries are received.
- Orient new clients, participants, families to the agency.
- Organize "Meet and Greets" for potential clients, including tours of the facilities, if required, and sit-down sessions to discuss available services and the referral process.
- Maintain positive working relationships with referral sources, coworkers, families, guardians, clients, etc.
- Maintain liaison with community partners, funding agencies, and referring agencies.
- Notes and Documentation
- Document all new referrals in assigned tracking systems.
- Accurately document all client encounters in the electronic Notes database system within 2 business days of service.
- Timely completion of monthly case notes and progress reports.
- Maintain and secure client files to preserve confidentiality.
- Ensure completeness and timeliness of case notes, contact notes, and assessment information.
- Participate in case record reviews.
- Ensure records are complete and accurate.
- Archive segments of the client record according to policy.
- Participate in departmental, Management Review Board, and staff meetings.
- Other duties as assigned.
SAFETY:
- Ensure a safe work environment for all assigned locations. Assist with the coordination of safety programs and procedures.
- Attend training and yearly review on the OSHA Bloodborne Pathogen Standard and Beacon's Exposure Control Program.
- Understand and follow the Standard and Exposure Control Program.
- Report any potentially dangerous situations to supervisor and Human Resources. Take steps necessary to protect clients and staff until the situation is corrected.
- Reinforce safe practices to clients and other staff.
- Attend safety training(s) as required.
- In the event of an industrial accident or illness, supervisors and/or managers will complete an accident investigation and report to Human Resources.
- Ensure compliance with all safety requirements - drills, inspections, etc. for CARF, DDD, and AHCCCS.

- Ability to be flexible and work in a team environment.
- High proficiency in Microsoft Office applications.
- Ability to provide accurate documentation of all client encounters.
QUALIFICATIONS:
- Education - HS/GED required. Associate degree or higher preferred.
- Demonstrated proficiency with Microsoft Outlook, Word, and Excel.
- Excellent organizational, communication, and time management skills.
- Proficiency in accurate, clear, and informative case note and report writing.
- Successfully pass the drug test and background check.
- Adult CPR/First Aid/AED certification, or able to obtain and maintain an Adult CPR/First Aid/AED Certification.
- Fingerprint clearance card, or able to obtain and maintain a fingerprint clearance card.
- Current vehicle registration and insurance coverage, if applicable.
- Provide proof of good driving (39-month MVR from AZ MVD Now), if applicable
- Have a valid Arizona Driver's License and pass the Beacon driving test, if applicable.
- Article IX and Prevention and Support certificate or able to obtain and maintain certification.
- Highly skilled with Microsoft Outlook, Word, PowerPoint, and Excel.
WORK HOURS AND SCHEDULE: Monday-Friday 5-20 hours per week
